VERIFICATION FORM

Once your service learning project is completely finished, fill out and submit the Verification Form. After this form is submitted, you will receive an email with the form as a separate file. Retain this form for your personal records as it will be useful when applying for colleges and future work experiences. Thank you for serving in your community! 

SERVICE LEARNING ACTIVITY FORM

You will need a proof document for you Verification form. If you do not have any pictures, forms from your supervisor, or anything else to use as proof then you can use this from. It will require your signature, the signature/s of your supervisor/s,  and the signature of the Service Learning Coordinator.
